Hartman Family Chiropractic & Wellness Center has a patient portal with our EHR system so you can conveniently complete your health history and medical consents conveniently in your home or office before you even arrive at our chiropractic office. It is important to us to have a full idea of your health and goals as well as if you have any questions or concerns.
If you struggle with computers, one of our dynamic staff members will be happy to assist you in our office entering your information or we can provide a paper copy as a last resort.
